The Meridian Medicaid Plan is a Medicaid managed care plan in Illinois. It is open to everyone in HealthChoice Illinois. Most people who get Medicaid will be enrolled in HealthChoice Illinois.

After you apply for Medicaid, follow these steps to enroll in Meridian:
Get started
After you are approved for medical benefits, you will get an enrollment packet in the mail. This packet will explain how to enroll in a managed care plan. Read it carefully.
Select a primary care provider
Before you enroll, you will need to select a primary care provider (PCP). This provider will be your go-to for all of your care needs.
Enroll
Once you have found a PCP in the Meridian network, it’s time to enroll. Use the simple online portal to pick Meridian as your health plan.
Say hello to Meridian
After you enroll in our plan, we will send you your member ID card in the mail. You will also get your member handbook in the mail.
Prefer to enroll in Meridian over the phone? Follow Step 2 to pick a PCP. Then call Illinois Client Enrollment Services at 1-877-912-8880 (TTY: 1-866-565-8576). You can call Monday to Friday, 8:00 a.m. to 6:00 p.m. The call is free.
